Difference in hemoglobin between smokers and non-smokers

흡연군과 비흡연군의 혈색소치의 차이 비교 -건강검진에 참여한 20세 이상의 남성을 대상으로-

Abstract

Background: The hemoglobin is a basic tool in diagnosing anemia. Not many studies compared hemoglobin between non-smokers and smokers in Korea and they did not differentiate hemoglobin between males and females. This study was conducted to show the difference of hemoglobin between non-smokers and smokers in male population and factors affecting hemoglobin. Methods: We gathered 313 men who underwent periodic health examination from May to July 2001 in a general hospital. The relationship between cigarette smoking and hemoglobin concentration was examined by comparing the means of hemoglobin between two groups and through multiple regression analysis. Results: The mean of hemoglobin was significantly higher in smokers than in non-smokers, especially in smokers with more than 1 pack per day. The factors that influence the hemoglobin level were age, the amount per day, and the total amount they smoked in the past(pack-year). Daily cigarette smoking seemed to cause a general upwared shift of hemoglobin distribution curve, while age and pack-year on the opposite. Conclusion: This study suggests that hemoglobin cutoff values should be adjusted for smokers and one must consider the patient's age to compensate for masking effect of smoking and aging on detection of anemia.

연구배경: 우리 나라 15세 이상 남자의 1995년 흡연률은 61%로, 흡연은 한국 남성 건강에 많은 영향을 미치고 있다. 외국 논문에 흡연에 의한 혈색소의 증가가 보고되었고, 특히 담배를 한 갑 이상 피우는 흡연군에서 혈색소치 증가가 보고된 바 있다. 우리 나라에서도 흡연이 혈색소치에 변수 요인으로 작용을 한다는 내용은 보고된 바는 있으나, 남성과 여성을 구분해서 혈색소치를 제시한 논문은 없었다. 본 연구는 흡연의 여부에 따른 혈색소치의 차이를 알아보기 위한 목적으로 수행하였다. 방법: 2001년 5월 1일부터 7월 30일까지 3개월간 춘천시내 모 대학병원 건강증진 센터를 건강검진 목적으로 방문한 20세 이상의 남성 313명을 대상으로 하였다. 결과: 남자의 경우 비 흡연군에서의 혈색소치는 $14.60{\pm}0.92$ g/dl 였고, 하루 한 갑 이하로 담배를 피는 흡연군의 경우 혈색소치는 $14.81{\pm}0.90$ g/dl, 1갑 이상 담배를 피는 흡연군 경우 혈색소치는 $15.19{\pm}0.84$ g/dl였다. 다중회귀분석에서 혈색소에 나이, 하루 흡연량, 체질량지수가 영향을 미쳤고, 나이와 일일흡연량, 흡연 연수를 보정한 상태에서도 혈색소와 흡연의 양과의 연관관계가 있었다. 결론: 남성에서 나이와 흡연이 혈색소치와 연관성이 높으므로, 흡연을 하는 남성에서 혈색소치에 의한 빈혈 진단시 나이와 흡연량에 따른 수치 변화를 고려해야 할 것으로 생각된다.
