The Evaluation of Contrast Sensitivity and Glare after LASIK Using Flying Spot Mode Laser According to Pupil Diameter and Refractive Error

Abstract

Purpose: To study the effect of LASIK using flying spot mode laser on contrast sensitivity and glare according to pupil diameter and preoperative refractive error. Methods : Fifty seven eyes of 42 patients had LASIK using MEL 70 G-scan (Asclepion Meditec, Germany) to correct myopia ranging from -2.50 to -10.50 diopter (D). The contrast and glare sensitivity were measured preoperatively and I week, 1 month and 3 months after LASIK using CGT-l000 (Takagi, Japan). Results: The mean preoperative spherical equivalent (SE) was -5.4${\pm}$2.9D. The mean pupil diameter was 5.95${\pm}$0.46mm in scotopic condition. Contrast sensitivity was significantly reduced I month after LASIK, depending on pupil diameter and degree of preoperative myopia. However, values returned to preoperative figures thereafter (P<0.05). Glare sensitivity was improved at all frequencies 3 months after LASIK, and most significantly at high frequency (P=.031). Conclusions: LASIK using employing the flying spot mode laser can effectively accomplish refractive correction without reduction of preoperative contrast and glare sensitivity values.

목적 : Flying spot mode lasser를 이용한 라식수술이 동공크기와 술 전 굴절력에 따라 대비감도 (Contrast sensitivity)와 눈부심 (Glare)에 미치는 영향을 알아보고자 하였다. 대상과 방법 : 2001년 4월부터 10월까지 flying spot mode laser (Mel 70 G-scan, Asclepion Meditec, Germany)로 라식 수술을 받았던 근시환자 42명 57안을 대상으로 술 전의 평균구면굴절력과 암실에서의 동공크기에 따라 CGT-1000 (T맘햐, Japan)을 이용하여 술 전과 술 후 1주, 1개월, 3개월째에 대비감도와 눈부심을 측정하였다. 결과 : 전체환자의 평균구면굴절력은 -5.4${\pm}$209 Diopter 였으며 암실에서의 평균동공크기는 5.95${\pm}$0.46 mm이었다. 대비감도는 intermediate 와 high frequency에서 술 전 구면굴절력이 심할수록, 동공크기가 클수록 술 후 1개월까지 유의한 감소를 보인 반면 (p<0.05), 눈부심 감도는 술 후 3개월째에 모든 frequency에서 술 전 수치에 비해 증가가 되었으며 특히 -6.25D이상인 군에서는 high frequency에서 유의한 향상이 관찰되었다(p=.031). 결론 : flying spot mode laser를 이용한 라식수술은 술 전과 비교하여 대비감도나 눈부심감도가 저하되지 않는 유용한 굴절교정수술로 사료된다.
