Diagnosis of Burst Fracture of the Spine on Plain Radiographs Using the $Wintopo^{\circledR}$ Program

Abstract

Purpose : To evaluate the usefulness of plain lateral radiographs processed by the ;$Wintopo^{\circledR}program (SoftSoft.net-$^{TM}$, version 1.11, USA) for a picture archiving communication system (PACS) for the diagnosis of bursting fracture. Materials and Methods : From January 2001 to April 2002, 24 cases of thoracic and lumbar spine fractures with available computed tomographs were evaluated. Lateral plain radiographs were reviewed by five orthopedic residents, and then were saved as bmp files in a picture archiving communication system. The borderline posterior body in the lateral plain radiographs was defined using the vector image in the ;$Wintopo^{\circledR}program. After processing, radiographs were re-evaluated. Results : In all participants, the sensitivity of the diagnosis of bursting fracture was significantly improved after ;$Wintopo^{\circledR}program processing. Although the specificity and positive predictive value were improved by processing, this was not statistically significant. Conclusion : ;$Wintopo^{\circledR}program processing of plain lateral radiographs of the spine could improve the sensitivity of diagnosis for bursting fracture in acute spinal injury.

목 적 : 영상 기록 전달 체계(picture archiving communication system, PACS)에서의 척추 측면 방사선 사진을 인터넷상의 무료 프로그램인 $Wintopo^{\circledR}$ (Soft-$^{TM}$ version 1.11, USA)를 이용해 처리한 후 급성 척추체 방출성 골절의 진단에 도움이 되는지 알아보았다. 대상 및 방법 : 2001년 1월부터 2002년 4월까지 컴퓨터 단층 촬영 사진이 있는 24예의 흉추부 및 요추부 골절을 대상으로 하였다. 5명의 정형외과 전공의 가 단순 측면 방사선 사진에 대하여 압박 골절 또는 방출성 골절 여부를 판독하게 한 후에, 대상 환자의 PACS 상의 단순 방사선 측면 사진을 bmp 파일로 전환 후 $Wintopo^{\circledR}$프로그램에 있는 벡터 이미지를 이용해 측면 단순 방사선 사진상의 후방 추체선을 탐색하여 검출하게 한 후 다시 판독을 하도록 하였다. 결과 : 모든 검사자에서 방출성 골절의 진단에 대한 민감도가 $Wintopo^{\circledR}$프로그램으로 처리한 후에 통계학적으로 의미 있게 향상되었으며, 특이도와 양성 예측도도 증가하였으나 통계학적 의의를 가지지는 못하였다. 결론 : $Wintopo^{\circledR}$프로그램을 이용하면 급성 척추체 손상 시 단순 방사선 사진을 이용한 방출성 골절 진단의 민감도를 높일 수 있을 것으로 생각된다.
