Abstract

Objective : We got a good effect on one patient who was managed a medical dispute. She icreased low back pain and right lower extremity pain after acupuncture treatment in a local oriental medicine hospital. Right lower extremity pain appeared allodynia and hyperalgesia. We would like to discuss with this case applied Cannon's low of denervation supersensitivity and Dr. Gunn's radiculopathic model. Methods : We used mainly a acupuncture treatment with a electro-acupuncture in the paraspinal acupuncture points. And apitoxin, herbal medicine. Results : Patients chief complain is the right lower extremity allodynia an hyperalgesia. Its onset was 10.7.2002. She had received the various treatments (a physical therapy in a orthopedic hospital and a acupuncture treatment in a local oriental hospital, e.t.c.) since 10.7.2002. We took L-Spine CT in 27.1.2003. And we found HNP, right paracentral extrusion type, L4-5, L4-5 central canal stenosis. The Right lower extremity induced a severe pain after a very soft touch like contacting and edge of bed. This symptom disappeeared after out treatment. Visual analogue scale changed from 8 to 1. and Pain Disability Index showed improved change. Conclusion : A acupuncture into a abnomal tissue with denervation supersensitivity can induce allodynia and hyperalgesia. The patients increasing pain after a acupuncture treatment (This managed a medical dispute.) caused the denervation supersensitivity by L4-5 HNP. The Cannon's law explained this denervation supersensitivity. L4-5 HNP induced the denervation supersensitivity in the solved the denervation supersensitivity by a acupuncture treatment in the paraspinal acupuncture points. This can remove the tightness of paraspinal muscle. And this can decrease denervation supersensitivity. So this can decrease allodynia and hyperalgesia. The acupuncture treatment in the paraspinal acupuncture points can be a good treatment in the denervation supersensitivity according to Cannon's law. Whenever We treat the patient of denervaton supersensitivity of radiculopathic model, we must be careful about misunderstanding medical dispute.
