Impact of Dry Mouth on Orofacial Pain Symptoms in the Korean Elderly

Abstract

The aims of this study were to investigate the impact of dry mouth on orofacial pain symptoms m the Korean elderly population, and to characterize the relationship between dry mouth and orofacial pain symptoms Telephone interviews were conducted on a stratified random sample of 1,012 people aged 55 years or older from a cohort of 4,342 elderly Koreans who had previously participated m the Korean National interview Survey of Oral Health Status (KNISOH) in 2000 Our questionnaire measured dryness-related symptoms and behaviors as well as assessed the prevalence and intensity during the most recent SIX months of five orofacial pain symptoms Jaw joint pain, facial pain, tooth pain, oral sores, and burning mouth. The prevalence of all five orofacial pain symptoms included m this survey was significantly higher for the dry-mouth group than for their non-dry-mouth counterparts. moreover, dry mouth emerged as the strongest risk factor for orofacial pain symptoms, in particular, the risk of burning mouth increased 4,35 times in cases of dry mouth (p<0.001) Multiple linear regression analysis also revealed that dry mouth affected orofacial pain intensity with the owner of medications (coefficient 747, 95% CI 184-13 10) For respondents who complained of concomitant dry mouth and orofacial pain, the Pearson's correction (0.52) between pain intensity and dry mouth seventy was significant at the p<0,001 level. most significantly, this study found dry mouth to be a common symptom and the strongest risk factor for other orofacial pain symptoms, generally exaggerating the intensity of pain Dry mouth IS a substantial health problem for the Korean elderly population.

본 연구의 목적은 한국 노령인구에서 구강건조증이 다양한 구강안변 동통에 미치는 영향을 조사하고, 구강건조증과 구강안변 동통과의 관계를 분석하여 노인의 구강건강과 삶의 질 향상에 기여하는데 있다. 한국에 거주하는 55세 이상 성인남녀 4,342 병의 표본 집단에서 지역, 연령, 성별, 무작위 층화추출법으로 표본 추출된 1,012 명 을 대상으로 구강건조증과 구강안변 동통에 관한 전화설문을 시행하였다 구강건조증과 관련된 증상이나 행동 및 그 심도, 구강안면 통통 ( 턱관절 통증 , 안면통증 , 치아통증, 구강 내 연조직 병소로 인한 통증, 구강 내 작열감 유무 및 통증의 심도, 그 밖의 원인요소, 즉 연령, 성별, 복용약물, 흡연 등의 변수들과의 관계를 분석하여 다음과 같은 결과를 얻었다. 1. 5개의 구강안연 동통 양상의 유병 률은 구강건조 감을 호소하는 군에서 비교군 보다 통계적으로 유의하게 높게 나타났다 (p<0.001) 2 구강건조 증은 구강안면 동통의 가장 영향력 있는 위험인자로 나타났으며 , 특히 구강 내 작열감은 구강건조 증으로 인해 유병 율이 4.35배 증가하였다 (95%CI; 2.45-7.73) 3. 동통의 심도는 구강건조 증 (Coeffcient, 7.47, 95%CI; 1.84-13.10) 과 복용약물의 수에 영향을 받는 것으로 나타났으며, 또한 동통의 심도는 구강건조 증의 심도와 유의한 상관관계를 나타내었다 (Pearson's correlation, 0.52, p<0.001) 구강건조 증은 노령인구에서 유발되는 흔한 증상으로 구강안변 동통의 유병률에 영향을 마치는 위험요소이며, 동통의 심도를 악화시킴으로써 노령인구의 구강건강에 중요한 문제로 제기되고 있다.
