Orofacial Evaluation of Tension-type Headache in Dentistry

긴장성 두통의 치의학적 평가

Abstract

This study was designed to evaluate dental features of tension-type headache(TTH) closely relating with orofacial region disorders Especially, Joint pam, trismus, and joint sound, known as main symptoms of temporomandibular disorders(TMD), were examined In patients with TTH Patients with TTH(N=60) having headache and orofacial pain were recruited at Department of Oral Medicine, Kyung Hee University Dental Hospital in two months Demographic findings(age, gender, etc) and several measures(visual analogue scale, pain site, oral habit, etc) were evaluated Statistical(Kernel release ver 55, StatSoft Inc, Tulsa OK, USA) was used for data analysis. 1. 25.0% of TTH patients are male and 75.0% are female Mean age is 31.3$\pm$12.7 years(male 28.9$\pm$11.6, female 32.1$\pm$13.1). 2. Chief complaint sites of TTH patient were predominant both head and orofacial region(91.7%) rather than head only(8.3%) The main sites were temporal area 93.3%, masseter area 75.0%, and TMJ area 36.7%, in order. 3. 33.3% of TTH patients had oral habits, of winch 38.1% was bruxism, 38.1% clenching, 19.0% bruxism and clenching, and 4.8% the other habit. 4. 63.3% of TTH patients had Joint and muscle pam Mean visual analog scale score was 59.2% Pam intensity b palpation was medium The sites were head region exciting temporal muscle(51.7%), TMJ region(46.7%), and orofacial region including temporal muscle(66.7%). 5. Mean active range of motion of mouth m TTH patients was 37.1$\pm$9.0 mm Mean passive range of motion was 43.5$\pm$7.1 mm Mean difference wass 6 4$\pm$5 3 mm. 6. Subjective and objective Joint sound of TTH patients was shown as 38.3% and 25%, respectively subjective Joint sound was more, but not significant.(p=0.13) Patients with TTH had so many dental symptoms Therefore, the dental exploration and management should be important for patients with TTH

긴장성두통이 발생 부위나 발현 양상에 있어서 구강인면영역과 밀접한 관련이 있으므로 긴장성두통에 관한 역학 및 그 임상적 특정을 치의학적으로 평가하며 특히 구강안면통증의 대표적인 측두하악장애의 주된 승상은 악관절통증, 개구장애 그리고 악관절음인데, 긴장성두통환자의 경우 나타나는 증상과도 상호 관련이 있는지의 여부도 확인해 보기 위하여 두부 및 구강안면부의 통증을 주소로 내원한 환자 60명을 대상으로 성별, 연령, 발병일 등의 기본사항과 시각유추통증척도, 통증발현부위, 구강악습관 등의 보조사항 그리고 악관절통증, 개구량, 악관절음 등의 측두하악장애 기준사항을 조사하여 통제 처리 후 다음과 같은 연구 성적을 얻었다. 1. 긴장성두통환자의 남여 성비는 각각 25.0%의 75.0%로 여성에서 뚜렷하게 호발하였고, 평균연령은 31.3$\pm$12.7세였으며, 남상 28.9$\pm$11.6세, 여성 32.1$\pm$13.1세였다. 2. 긴장성두통환자의 주소는 두부에만 발현된 경우보다 두부와 구강안면부에서 동시에 발현된 경우가 91.7%로 우세하였으며, 측두부 93.3%, 교근부 75%, 악관질부 36.7% 순서였다. 3. 긴장성두통환자는 구강악습관이 33.3%있었으며, 이 중 이갈이 38.1%, 이악물기 38.1%, 이갈이와 이악물기가 함께 있는 경우 19.0% 그리고 기타 4.8%였다. 4. 긴장성두통환자의 통증은 관절통과 근육통이 함께 있는 경우가 63.3% 로 가장 많았고, 시각유추통증직도는 평균 59.2%였으며, 통증의 강도는 근육촉진 시 측두부를 제외한 두부, 악관절부, 측두부를 포함한 안면부 모두에서 중등도로 각각 5l.7%, 46.7%, 66.7%였다. 5. 긴장성두통환자의 개구량은 주관적 개구량이 평균 37.1$\pm$9.0 mm, 객관적 개구량이 평균 43.5$\pm$7.1 mm, 개구량차이 평균은 64$\pm$53mm였다. 6. 긴장성두통환자의 악관절음은 주관적 관절음 38.3%, 객관적 관절음 25%로 주관적 관절음이 상대적으로 많았지만, 통계적으로 유의하지는 않았다. (p=0.13) 긴장성두통은 치의학적으로 관련성이 많았으며, 특히 측두하악장애와 임상적으로 상호 고려할 부분이 많았다. 따라서 긴장성두통의 진료에 실질적인 치의학적 접근이 요구되며, 향후 긴장성두통에 관한 계속적인 치의학적 연구기 필요 하리라 사료된다
