혈액투석 환자에서 발생한 중심정맥 협착증 - 혈관확장술 및 스텐트 삽입의 치료효과

Abstract

배 경 - 혈액투석 환자에서 발생하는 중심정맥 협착증은 전체 정맥 협착증의 약 17%로 비교적 드물게 발생되지만 말초혈관의 정맥 협착증에 비해 외과적 교정술이 어려워 대부분이 혈관중재술로 치료되고 있다. 외국에서는 혈액투석 환자에서 발생하는 중심정맥 협착증에 대해 많이 보고되어 있으나 국내에서는 이에 대한 보고가 많지 않다. 방 법 - 지속적인 혈액투석을 받고 있는 말기신부전증 환자에서 정맥조영술을 시행하여 중심정맥 협착증을 진단 받은 31명의 환자를 대상으로 하였다. 대상환자들의 의무기록을 통하여 중심정맥 협착증의 원인, 임상증세, 정맥조영술 소견, 그리고 혈관중재술의 치료효과를 후향적으로 조사하였다. 결 과 - 대상환자 31명 중 혈액투석을 위한 중심정맥 도관을 삽입했던 환자는 28명이었으며, 이 환자들에서 도관 삽입기간은 평균 41±23일이었다. 정맥 협착증의 임상 증상은 상지부종이 22예로 가장 많았고, 그 외 투석 중 정맥압 상승 10예, 급성 동정맥루 폐쇄 4예, 안면부종이 2예였다. 정맥조영술에서 중심정맥의 완전폐쇄를 보인 환자는 14명이었고, 부분협착을 보인 환자는 17명이었다. 정맥 협착증의 위치는 우측 쇄골하정맥 11명, 좌측 쇄골하정맥 7명, 무명정맥 9명, 상대정맥이 4명이었다. 총 31명 환자 중 26명에서 총 30회의 혈관확장술 또는 스텐트 삽입 등의 혈관중재술이 시행되었다. 시술시에 혈관파열 등의 합병증은 발생하지 않았고 초기 성공률은 96%였다. 1차 시술의 개통률은 6, 12, 24, 48개월에 각각 87.3%, 75.6%, 67.9%, 65.4%였으며, 재협착이 발생한 환자에 대한 재시술 후의 누적 개통률은 각각 96%, 90.6%, 74%, 72.8%였다. 결 론 - 혈관확장술 및 스텐트 삽입은 혈액투석 환자에서 발생하는 중심정맥 협착증의 치료로 안전하고 매우 효과적인 방법으로 사료된다.
