The Effect of Atropine on Myopic Progression in Children

Abstract

Purpose: To examine the effects of atropine therapy on myopic progression in myopic children. Methods: Fifty myopic patients, aged from 7 to 14 years and less than -5 diopters, visited our clinic for correction of refractive error from August 2001, and were followed up for at least 1 year. Of these 50 patients, the atropine therapy group was 23 and the control group was 27. Right eye cycloplegic refraction and axial length changes of the two groups were compared and analyzed. Results: There was a mean increase of myopia of 0.25$\pm$0.43 D/yr in the atropine therapy group and of 0.83$\pm$0.37 D/yr in the control group (P=0.000). There was a mean increase of axial length of 0.12$\pm$0.19 mm/yr in the atropine therapy group and 0.39$\pm$0.21 mm/yr in the control group (P=0.000). In refraction and axial length changes according to age, there was no statistically significant difference within the group, aged from 7 to 9 years. However, there was a statistically significant difference within the group, aged from 10 to 14 years. In refraction and axial length changes according to myopic grade, there were a statistically significant differences between the groups. Conclusions: Atropine therapy is effective in slowing the myopic progression in myopic children with refractive error less than -5 diopters.

목적 : 소아 근시환아를 대상으로 아트로핀 점안치료가 근시진행에 미치는 영향에 대해 알아보고자 하였다. 대상과 방법 : 2001년 8월부터 굴절이상 교정을 위해 소아안과 외래에 내원한 7세에서 14세 사이, -5D 미만의 굴절값을 가진 근시환아 중 일년 이상 경과관찰이 가능했던 50명의 환아를 대상으로 아트로핀을 점안한 치료군(23명)과 안경교정만 했던 대조군(27명)간에 우안의 조절마비하 굴절값과 안축장변화를 비교분석하였다. 결과 : 굴절변화에 있어 아트로핀 치료군은 0.25$\pm$0.43 D/yr, 대조군은 0.83$\pm$0.37 D/yr으로 통계적으로 의미있는 근시진행 억제효과를 보였고(P=0.000), 안축장변화에 있어서도 치료군은 0.12$\pm$0.19 mm/yr, 대조군은 0.39$\pm$0.21 mm/yr으로 의미있는 결과를 보였다(P=0.000). 나이에 따른 굴절변화 및 안축장변화를 보면 7~9세에서 통계적으로 유의한 차이는 보이지 않았지만, 10~14세에서는 통계적으로 유의한 차이를 보였다. 근시정도에 따른 굴절변화 및 안축장변화는 두 군간에 통계적으로 유의한 차이를 보였다. 결론 : 아트로핀이 -5D 미만의 소아 근시환아에서 근시진행 억제에 효과적으로 사용될 수 있을 것으로 사료된다.
