The Prevalence of Polycystic Ovary Syndrome in College Students from Seoul

Abstract

Background: Polycystic ovary syndrome (PCaS) is characterized by the presence of chronic anovulation and hyperandrogenism, The prevalence of pcas in premenopausal women of about 5- 10%. There may be significant ethnic and racial variations in the prevalence and clinical presentation of pcas. The current study was undertaken to determine the prevalence of pcas and to identify the clinical characteristics of Peas in Korea. Methods: From March 2003 to March 2004, medical students from Ewha Womans University were investigated for the prevalence of pcas. The evaluation included a history and physical examination, a modified Ferriman-Gallwey hirsutism score and screening for hyperandrogenemia. Peas was diagnosed by the presence of the following: I) oligomenorrhea, 2) hyperandrogenemia and/or hirsutism (a modified Fertiman-Gallwey score 2':6), and 3) the exclusion of related disorders. Hyperandrogenemia was defined as a free testosterone level above the 95th perceutile of that for normal cycling nonhirsute women. Results: Out of 492 students, 386 replied to the questionnaire and 203 students (19-31 yr of age) participated in the physical examination and blood sampling. Eumenorrhea without hirsutism was present in 133 students (65.5%), oligomenorrhea was present in 39 students (19.2%), hyperandrogenism was present in 21 students (10.3%) and hirsutism was present in 3 (1.5%) of the 203 women. The prevalence of pcas was 4.9% (10 of 203 students), and only 2 of 10 women with PCOS were hirsute. The mean body mass index of students with Peas was 22.7 $\pm$ 4.3 kg/m', and two women was obese. The plasma insulin levels during the 75 g oral glucose tolerance test in women with pcas was significantly higher when compared to the age and body mass index of the controls. Conclusion: The prevalence of pcas was 4.9% in college students from Seoul. This rate is similar to those rates reported by other investigators in Greece (6.7%) and United States(4%). Obesity and hirsutism are not common in Korean women with Peas. We are planning to survey a more diverse age groups to determine.

연구배경: 다낭성난소증후군(polycystic ovary syndrome, PCOS)은 만성 무배란과 안드로겐 과다를 특징으로 하는 가임 여성의 6~10%에서 발생하는 흔한 내분비질환이다. PCOS의 유병률은 인종에 따라 다를 것으로 생각되나 GUSWOrk지 한국인에서 PCOS의 유병률은 조사된 바 없는 실정으로 본 연구는 한국인에서 PCOS의 유병률을 결정하고 임상적인 표현형을 규명하는 예비연구의 일환으로 시행하였다. 방법: 이화여자대학교 의과대학 학생을 대상으로 2003년 의한 응답자에서 난포기 초기에 혈액검사와 신체계측을 하였다.PCOS의 진단은 1990년 NICHD conference의 기준에 의하여 선천성 부신증식증 및 쿠싱 증후군 등 고안드로겐증을 유발할 수 있는 내분비계 질환이 동반되지 않고 연 8회 이하의 희소월경과 고안드로겐혈증이 있는 경우로 하였다. 결과: 총 492명중 386명이 설문에 응답하였으며 203명이 혈액검사에 동의하였다. 203명의 연령은 19세에서 31세(평균 23$\pm$2세)였다. 정상군 133명(65.5%), 희소월경 39명(19.2%), 고안드로겐혈증 21명(10.3%), 조모증 3명(1.5%), 희소월경과 고안드로겐혈증이 있는 PCOS는 10명(4.9%)이었고, 그중 2명에서 조모증이 동반되었다. PCOS의 평균 BMI는 22.7 $\pm$4.3kg/$m^2$였으며 2명이 비만 (BMI$\geq$25kg/$m^2$)이었다. PCOS 10명 중 1명이 제2형 당뇨병이었고 정상내당능을 보인 9명은 경구 당부하검사 중 인슐린 농도가 연령과 체질량지수를 일치시킨 대조군에 비해 높았다. 결론: 서울지역 여자대학생에서 PCOS 유병률은 4.9%이며, 이는 미국 여성의 4%, 그리스 여성의 6.7%와 유사한 결과이다. 또한 한국여성의 PCOS는 조모증이 동반되지 않고 대부분 비만하지 않을 것으로 생각된다. 향후 본 교실에서는 이러한 예비결과를 바탕으로 다양한 연령층의 여성을 포함한 PCOS 유병률 조사 및 PCOS환자의 등록체계 구축을 추진 중이다.
