Abstract
Background : Nonalcoholic fatty liver disease (NAFLD) has recently been recognized to be one of the leading causes of chronic liver disease. Obesity and type 2 diabetes are associated strongly with NAFLD. Although often used to detect liver disease, sometimes the cause of elevated Alanine Transaminase (ALT) activity was unclear. In the absence of other causes, the author examined whether BMI and metabolic markers of NAFLD can explain the cause of abnormal ALT in Korean. Methods : From April 2002 to November 2003, 9997 persons visited health promotion center of Kyungpook National University Hospital and took health screening. After excluding persons with excessive alcohol consumption, abnormal $\gamma$-GTP, hepatitis B, hepatitis C, 8379 persons were included in this study. All patients underwent anthropometric measures, blood chemistry analysis and abdominal ultrasonographic exam. Resutls : Elevated ALT levels were found in 10% (835 persons) of the all subjects. In univariate analysis, factors associated with elevated ALT levels (p<0.05) included male sex, younger age (20-40 years), higher Body Mass Index (BMI), higher blood pressure, higher fasting blood glucose, higher triglyceride, lower HDL and more fatty change in ultrasonography. The proportion of elevated ALT with fatty change in ultrasonography was 57% and with higher BMI ($\geq$or=23 kg/m$^2$) was 79% in abnormal ALT group. In multivariate logistic regression study, total cholesterol and blood pressure were eleminated but sex, BMI, triglyceride level, ultrasonographic fatty change were strongly associated with elevated ALT level (p<0.01). Conclusion : In this study, unexplained ALT elevation was strongly associated with obesity, adiposity and other features of metabolic syndrome, and thus may represent nonalcoholic fatty liver disease. However, more research is needed for the definite clinical determinants of nonalcoholic fatty liver disease in Korean.
배경 : NAFLD는 서구에서 원인이 명확하지 않은 간경변증의 가장 중요한 원인으로 비만은 이 질환군의 가장 중요한 위험인자로 여겨지고 있다. 이 중NASH는 무증상의 NAFLD에서 간경변증으로의 진행에서 결정적인 중간단계로 적극적이고 적절한 대처를 필요로 한다. 비만과 제2형 당뇨병은 NAFLD와 밀접한 관계를 가진 위험인자로 알려져 있다. 간 질환의 진단에 가장 일반적인 검사임에도 불구하고 원인이 명확하지 않은 ALT의 증가를 흔히 외래에서 발견하게 되는데, 본 연구에서는 한국인에서 특별한 원인이 없는 ALT 증가요인으로 비만의 중요성을 확인하고 비만 환자에서 NAFLD로의 진행에 관계하는 결정인자들을 살펴보고자 하였다. 방법 : 2002년 4월 1일부터 2003년 11월 31일까지 건강검진을 목적으로 병원을 방문한 9,997명의 환자 중 문진을 통하여 과량의 음주력이 있는 환자나 $\gamma$-GTP의 상승이 보이는 환자나 HBs Ag(+)이거나, HCV ab(+) 등 의 명확한 간 질환이 의심되는 1,618명을 제외한 8,379명을 대상으로 하였다. 대상자 전원에 대해 신체계측 검사 ,혈압측정, 공복상태에서의 혈액검사 및 간 초음파 검사를 실시하여 ALT가 정상인 group (n=7544)과 높은(>40) group (n=835)으로 나누어 성별, 나이, BMI, waist-hip ratio, 혈압, 식전혈당, total cholesterol, HDL. TG 및 초음파상의 지방간의 정도를 비교 분석하였다. 결과 : ALT의 증가는 간 질환의 원인이 없는 전체의 10% (835명)에서 확인되었고, 두 그룹간의 각 요인들의 평균은 independent t-test상에 거의 모두 유의한 차이를 보였다(p<0.01). 또한 ALT 증가 환자에서 중등도 이상의 지방간 소견을 보이는 경우는 57%, BMI 23 이상의 비만 그룹이 79%로 분포의 차이를 보였다. 회귀분석결과 총 콜레스테롤 양과 혈압은 배제되었고, 남자20대에서 40대의 나이, BMI가 25보다 큰 경우, 중성지방 정도, 간초음파상의 지방간 소견들이 ALT 증가에 의미있게 기여하는 것으로 나타났다. 특히 초음파상의 중등도 이상의 지방간이 있는 경우는 그렇지 않은 경우에 비해 ALT가 상승할 가능성이 5배 이상 높았다(p<0.01). 결론 : 본 연구에서 바이러스와 알코올 등의 요인을 배제한 ALT의 상승에 있어서 비만과 대사성 질환의 위험인자들과의 연관성을 확인할 수 있었으며, 비만인구가 점차 늘어나는 현 추세를 감안할 때 비만 환자에서의 간 질환의 동반 여부에 대한 많은 관심이 필요함을 알 수 있었다. 또한 비만에서 NAFLD로의 진행에 관여하는 인자들에 관한 더 많은 다양한 연구가 필요할 것으로 생각된다.
