The Performance of Contracting-out Public Health Services

공공의료서비스 민간위탁의 성과에 관한 연구

Abstract

This study aims to evaluate the performance of contracting-out public health services. Since the publication of Privatization : The key to better government by Savas, privatization has been regarded as a panacea to government problems. The study asks whether the key also works in the cases of health services. Three contracting-out cases of local public hospitals are compared and analyzed. The results are as follows. First, the effect of saving on the government budget was insignificant on the whole. Only one case showed some budget reduction. Second, contracting-out fairly improved the profitability but decreased the public value of the hospitals. Third, hospitalization charges increased considerably. In short, the results show that the outcomes of contracting-out vary for public health services and that the promise of a small government is not easy to attain.

본 논문은 공공의료서비스의 경우에도 민간위탁이 더 효율적인 정부를 가져오는가라는 문제에 답하기 위해 1990년대 중반 이후 민간위탁된 세 곳의 지방공사의료원의 위탁성과를 비교ㆍ분석하였다. 연구대상으로 아직 그 위탁성과에 관한 연구가 별로 이루어진 바 없는 지방공사의료원 위탁 사례들을 선정하였으며, 기존의 공공의료기관 위탁 관련 연구들과는 달리 위탁으로 인한 정부와 소비자의 비용 부담의 변화도 고찰하였다. 세 지방공사의료원의 위탁성과를 평가한 결과, 민간위탁이 약속한 비용의 절감은 별로 나타나지 않았으며, 정부가 부담하는 비용과 소비자가 부담하는 비용이 오히려 위탁으로 상승하는 경우가 더 많았음을 확인할 수 있었다. 또한 대체로 위탁으로 수익성은 증대하였으나 공공성은 저해되었다는 결과가 나타났다.
