Abstract

Purpose : This study examined the factors affecting pain, the pattern of pain, as well as the intensity and duration after a total knee arthroplasty. Materials and Methods : As a prospective study, 69 osteoarthritic knee patients who underwent total knee arthroplasty from Jan. 2004 to Nov. 2004, were examined using a visual analogue scale (VAS) in order to determine the pain intensity, duration, pain character and pattern at 6 weeks, 3 months, 6 months and 1 year after surgery. We checked for possible factors such as height, body weight, body mass index (BMI), HSS score, deformity, range of motion preoperatively, prosthesis type and operation procedure intraoperatively, the HSS score and range of motion at postoperative one year. The Pearson correlation test and Kruskal-Wallis test were used for statistical analysis. Results : The mean preoperative and 1-year postoperative HSS score was 61.4 and 94.6, respectively. The height, body weight, body mass index (BMI), HSS score, deformity, range of motion, prosthesis type and surgical procedure were not related to the intensity and duration of the pain. The mean postoperative VAS score at 6 weeks and 3, 6 and 12 months was 4.3${\pm}$.0, 2.7${\pm}$0.9, 1.6${\pm}$0.4, 1.3${\pm}$0.4,respectively. Five patients had mild pain until postoperative one year. Conclusion : The pain after total knee arthroplasty disappeared with time. However, mild knee pain sometimes remained even 1 year after surgery.

목적: 슬관절 전치환술 후 슬관절 통증의 연관인자, 양상, 통증의 정도와 지속 기간을 알아보고자 하였다. 대상 및 방법: 2004년 1월부터 2004년 11월까지 슬관절 전치환술을 시행한 환자 69명을 대상으로 전향적인 방법을 통하여 술 후 1년 간의 슬관절 통증을 분석하였다. 일상에서 느끼는 통증의 정도와 활동양상에 따른 통증의 정도를 visual analogue scale (VAS)을 이용하여, 수술 후 6주, 3개월, 6개월, 12개월에 외래방문 시 조사하였다. 수술 전 HSS 점수, 관절 변형 및 슬관절 운동범위와 수술시 치환물의 종류 및 수술과정, 술 후 1년에 HSS 점수, 슬관절 운동범위를 분석하여, 통증의 지속기간, 통증의 양상과 연관성을 통계학적으로 분석하였다. 결과: 수술 전 HSS 점수는 평균 61.4점에서 수술 6개월 후 평균 90.2점, 수술 1년 후 94.6점으로 향상되었다. 환자의 키, 체중, 체질량지수, 슬관절 변형정도, 수술 전후 슬관절 운동범위, 치환물의 종류 및 수술법 등은 통증의 변화정도와 연관성이 없었다. 통증의 정도는 visual analoge scale상 수술 후 6주에는 모두에서 조금이라도 통증을 호소하였으며, 이들의 VAS 점수는 4.3${\pm}$1.0점, 3개월째는 58명으로 이들의 평균 점수는 2.7${\pm}$0.9점, 6개월째는 9명으로 1.6${\pm}$0.4점, 1년에는 5명으로 1.3${\pm}$0.4점이었다. 결론: 슬관절 전치환술 후 발생하는 통증은 대부분 수술 후 시간이 지나면서 줄어드나, 술 후 1년까지 통증이 지속되는 경우도 있다.
