Effects of Walking and Physical Activity on Glucose Regulation among Type 2 Diabetics

Abstract

Background: Physical activity, especially walking is strongly recommended to control blood glucose among type 2 diabetic patients. Furthermore, physical activity is one of the most important tools to prevent secondary diabetes complications among type 2 diabetic patients such as retinopathy, nephropathy, neuropathy etc. The purpose of the study was to examine the association between the level of walking and physical activity and glucose control among Korean adults with type 2 diabetes.Methods: A total of 250 patients with type 2 diabetes (98 males and 152 females) were recruited (mean age = 62.1 ± 10.2 years) in the current study. The height, weight, waist and hip circumference were measured, and the level of physical activity and total walking hour were measured by physical activity scale for elderly (PASE). High density lipoprotein cholesterol (HDL-C), total cholesterol, triglyceride, fasting glucose and oral glucose tolerance test, creatinine, uric acid, total protein, albumin, hemoglobin A1c were measured.Results: After adjusting for potential covariates such as age, education, occupation income, smoking, and drinking, male patients who spent least time in walking were more likely to have 2 hour serum glucose level in oral glucose tolerance above 200 mg/dL than counterparts who spent most time in walking with age adjusted (Relative Risk (RR) = 11.75, 95% Confidence Interval (CI) = 1.94-71.00). Male patients who were in the least active group were 5.92 time (95% CI = 1.39-25.28) more likely to have 2 hour serum glucose level in oral glucose tolerance over 200 mg/dL than counterparts in the most active group. However, there was no significant finding in females.Conclusions: The current study showed that physical activity and walking are effective method to maintain glucose tolerance among type 2 diabetic male patients. 연구배경: 신체활동 참여는, 특히 걷기가 제2형 당뇨병환자의 혈당조절 향상을 위해 권장되고 있다. 특히 당뇨병환자들의 합병증인, 안구질환, 신장질환, 신경질환 예방을 위해 신체활동은 필수적이다. 따라서 본 연구는 제2형 당뇨병환자들의 신체활동, 걷기와 혈당조절과의 관계를 알아보고자 한다.방법: 본 연구에 총 250명의 성인 당뇨병환자가 참여하였다(남 98명, 여 152명 평균나이 62.1세). 키, 몸무게, 허리둘레, 엉덩이 둘레 등 신체계측을 실시하였고, 설문지를 사용 신체활동량과 걷기시간을 측정하였다. 또한 고밀도 지단백 콜레스테롤, 총 콜레스테롤, 중성지방, 혈당, 요산, 단백질, 알부민, 당화혈색소 등을 측정하였다.결과: 나이, 학력, 직업, 소득, 흡연 등의 변인을 통제한 후, 걷기시간과 신체활동량 따른 경구 당부하 2시간 혈당을 관찰 하였을 때, 남자 환자들의 경우 걷기시간이 가장 적은 그룹은 걷기시간이 가장 많은 그룹에 비해 경구 당부하 2시간 혈당이 200 mg/dL 이상이 될 위험도가 11.75배 높은 것으로 보고되었고, 신체활동량이 가장 적은 그룹의 남자환자가 신체활동량이 가장 많은 그룹의 환자들에 비해 경구 당부하 2시간 혈당이 200 mg/dL 이상이 될 위험도가 5.92배 높은 것으로 보고되었다. 그러나 여성 환자들에게서는 유의한 차이가 없었다.
