Four cases of toxic liver injury associated with Dictamnus dasycarpus

백선 복용 후 발생한 독성 간손상 4예

Abstract

백선 달인 물을 복용한 뒤 발생한 급성 간염 4증례를 보고하고자 한다. 환자들의 나이의 중앙값은 60세였으며 공통되는 증상은 황달과 전신쇠약감이었다. 4명의 환자 모두에서 내원 전 5년 동안 음주력은 없었고, A형, B형, C형바이러스간염을 시사하는 표지자도 발견되지 않았으며, 복부초음파검사에서 총수담관의 확장 또한 관찰되지 않았다. 첫 번째와 두 번째 증례는 모녀 관계로서 백선 달인 물을 함께 나누어 복용하였으며, 세 번째와 네 번째 증례의 경우에는 산발적으로 나타났다. 환자들은 모두 백선 뿌리를 달인 용액을 평균 2.5개월 동안 하루 5∼6회씩 복용하였던 적이 있었다. 백선에 의한 간손상은 약제 중단 후 1개월 이내에 회복되고 양성 경과를 보여준다. 본 사례들로 미루어 백선은 농촌 지역에서 급성 간염을 일으키는 원인 물질이 될 수 있는 것으로 판단된다.
