A Study on Quality of Life and Economic Burden in Korean Patients with Atopic Dermatitis and Their Parents

Abstract

Background: Because of the chronic course and highly disfigured skin lesions, atopic dermatitis (AD) has a substantial impact on quality of life (QoL). Moreover, previous studies assessing the financial cost of AD have shown that it represents a notable economic burden. However, data about QoL and the economic burden on Korean patients with AD are limited. Objective: The purpose of this study is to assess the impact of AD on QoL as well as the economic burden in Korean patients and their parents. Methods: A questionnaire specifically designed by our clinical experience was used to determine the ways in which AD affects the lives of the patients and/or their parents. Eighty-two patients and 47 parents completed the self-administrated questionnaires, including the questions on multi-dimensional aspects of QoL. They were also surveyed to find how much money they have spent in the treatment of AD during the last 6 months. In addition, for each patient the disease severity was evaluated by using the SCORAD index. Results: The QoL data revealed that AD had significantly negative effects on QoL through all aspects of life including daily living activities (37.2), psychological status (30.0) and social functioning (23.7). The QoL scores significantly increase in proportion to the increasing severity of AD (p<0.0001). With regard to daily living activities, a major stress-causing factor was restriction on cosmetic use (50.0). For psychological status, 91.5% of patients reported that they feel deeply nervous about the likelihood of their offspring having AD in future (62.2). Regarding social functioning, it was told that AD patients had difficulties in developing relationships (28.8). AD parents also had high level of stress associated with modifying lifestyles (42.1). The estimated cost related to AD has the positive correlation with the severity of AD. Of importance, the average direct medical cost corresponds to only 29% of the total cost, while the indirect cost represents 71% of the total cost. Conclusion: These results demonstrate that AD has a significant impact on the various components of QoL in Korean patients and their parents. Moreover, AD imposes a considerable economic burden on the family budget. (Korean J Dermatol 2008;46(2):160∼170)
