The Relating Factor and Quality of Life of Overactive Bladder in Adults

성인의 과민성방광증후군의 관련 요인 및 삶의 질

Abstract

Background: Overactive bladder syndrome (OAB) is a common disorder in primary care that affects negatively on the quality of life of outpatients. However, little information is available on related factors and the quality of life in adults with overactive bladder. The purpose of this study was to find related factors of overactive bladder and to measure the quality of life in adults with overactive bladder. Methods: Among the people who visited a hospital in Seoul and Chung-ju for a health examination or primary care between October 1 and December 31, 2008, we selected 327 participants over 20 years old. We measured their Overactive Bladder Questionnaire (OAB-q), took medical history and reviewed their demographic data. The relationship between OAB, other independent variables and health related quality of life (HRQL) were analyzed. Results: Of 327 respondents, the prevalence of OAB in adults was 38 (11.6%). The factors related to OAB were age (P = 0.007), history of prescription for congestive heart failure (P = 0.023), benign prostatic hyperplasia (BPH) (P = 0.002), and depression (P = 0.008). OAB was signifi cantly associated with BPH medication (P = 0.042; OR = 8.757) and depression medication (P = 0.005; OR = 9.977) in multivariable logistic regression analysis. In each T-test analysis, OAB decreased in HRQL. Conclusion: OAB is a common disorder in adults, history of BPH medication and depression medication is more common in OAB. The symptoms that were suggestive of OAB were affected negatively on the quality of life in adults. Effective health care polices and prompt management of OAB should be implemented.

연구배경: 과민성방광증후군(overactive bladder syndrome)은 일차 의료에서 흔하게 접할 수 있는 질환이며 과민성방광증후군의 증상은 삶의 질에 부정적 영향을 미치게 된다. 그러나 성인에서의 과민성방광증후군과 관련된 인자 및 삶의 질에 대한 정보가 미흡한 실정이다. 따라서 본 연구에서는 성인에서의 과민성방광증후군의 관련 인자를 찾고, 과민성방광증후군의 삶의 질을 측정하고자 한다. 방법: 2008년 10월 1일부터 12월 31일까지 서울 및 충주 일개 대학병원을 방문한 20세 이상의 환자 396명을 대상으로 인구학적 특성을 묻는 설문지와 Overactive Bladder Questionnaire (OAB-q)를 시행하였고, 최종적으로 설문 누락이나 통계 불가한 69명(17.4%)을 제외한 327명을 대상으로 과민성방광증후군의 관련 인자 및 삶의 질과의 연관성에 대하여 통계 분석하였다. 결과: 총 327명 중, 과민성방광증후군으로 진단된 수는 38(11.6%)명이었다. 과민성방광증후군은 연령대(P=0.007), 심부전증 약제 복용력(P=0.023), 전립선비대증 약제 복용력(P=0.002) 및 우울증 약제 복용력(P=0.008) 등과 관련 있는 것으로 나왔으며, 이를 연관 변수로 보정한 결과 전립선비대증 약제 복용력(P=0.042; OR=8.757) 및 우울증 약제 복용력(P=0.005; OR=9.977)과 연관성 있는 것으로 나왔다. 과민성방광증후군으로 진단된 군에서 정상인 군에 비하여 삶의 질이 확실히 감소함을 알 수 있었다. 결론: 성인에서 과민성방광증후군은 일반적으로 흔하게 접할 수 있는 질병이며, 전립선비대증 약제 복용력 및 우울증 약제 복용력이 있는 경우 더 흔하게 관찰되었고, 과민성방광증후군과 관련된 증상들로 인하여 삶의 질의 감소가 나타났다. 과민성방광증후군으로 고통 받는 환자들을 적절히 파악하고, 관련인자를 분석하여 예방책과 치료에 관심을 가져야 할 것이다.
