간기능검사의 이해와 적용

Abstract

간기능검사는 간담도계 질환의 선별에 효과적이고 감별 진단의 방향을 제시하며, 질병의 중증도와 치료 반응 평가에 유용하다. 그러나 진행된 간질환에서 정상일 수 있고, 간질환 이외의 질환에서 이상 소견을 나타낼 수 있으며, 간기능검사만으로 구체적 질환을 직접적으로 알 수 없다는 한계가 있다. 이를 보완하기 위하여 자세한 병력 청취와 신체 검진을 통하여 간질환의 증상과 징후를 평가하는 것이 필수적이다. 간기능검사 이상을 몇 가지 유형(간세포손상형, 담즙 정체형, 침윤형)으로 분류하는 것이 감별 진단의 범위를 좁히고 진단에 특이한 검사를 선택하여 구체적 질환을 진단하는데 도움을 줄 수 있다.
