Levels of Severity in Dry Eye Syndrome According to Delphi Panel Classification

Abstract

Purpose: To investigate levels of severity in dry eye syndrome according to Delphi panel classification. Methods: Three hundred and twenty-two eyes of 166 patients with dry eye syndrome that did not have coexisting lid margin disease or altered tear distribution and clearance were categorized into 1 of 4 levels of severity, according to the symptoms and signs in the Delphi panel classification. A symptom score was proposed depending on the frequency and impact on the quality of life. Degree of corneal staining using Lissamin green was categorized using the Oxford Scheme. In addition, measurement of the tear film break-up time (BUT) and Schirmer test were performed. Results: According to the levels of severity in the Delphi panel classification, 120 eyes (37.26%) were categorized as level 1, 166 eyes (51.55%) as level 2, 30 eyes (9.31%) as level 3, and 6 eyes (1.86%) as level 4. Negative correlations were observed between the levels of severity, BUT, and measurements of the Schirmer test (p<0.001). Conclusions: Of the 166 eyes with dry eye syndrome that did not have coexisting lid margin disease and altered tear distribution and clearance, 51.55% of eyes were classified as level 2, and 88.81% of eyes were classified as level 1 and level 2.

목적: 건성안 증후군 환자를 Delphi panel의 중증도 level에 따라 분류하고자 하였다. 대상과 방법: 안검염을 동반하지 않고 눈물 배포와 배출의 이상을 동반하지 않는 건성안 증후군 환자 166명 322안을 대상으로 Delphi panel에서 제시된 중증도 level에 따라 중증도를 분류하였다. 증상은 일상생활에 불편을 주는 정도에 따라 분류하였고, 각막 염색 정도는 Lissamin green 염색을 이용하여 Oxford scheme에 따라 분류하였다. 추가적인 진단검사로 눈물막 파괴시간측정 및 쉬르머검사를 시행하였다. 결과: 건성안을 분류한 결과 level 1이 120안(37.26%), level 2가 166안(51.55%), level 3가 30안(9.31%) 및 level 4가 6안(1.86%)으로 분류되었다. 중증도 level과 눈물막 파괴시간 및 쉬르머검사값과는 음의 상관관계를 보였다(p<0.001). 결론: Delphi panel의 기준에 따라 건성안의 중증도를 분류한 결과 level 2에 속한 경우가 51.55%로 가장 많았으며, level 1과 2에 속한 경우가 전체의 88.81%에 해당하였다.
