한국 성인의 지역적 요인에 따른 치주 상태 비교

Abstract

연구목적. 치주질환은 성인의 구강 상태에 영향을 미치는 주요한 요인으로, 최근 사회경제적 요인이나 지역적 차이에 따른 구강 상태에 대한 관심이 증가하고 있다. 본 연구는 2000년 전국민 구강건강실태조사 자료를 활용하여 한국 성인을 대상으로 거주 지역 요인과 치주 상태와의 관련성을 알아보고자 하였다. 연구방법. 국민구강건강실태조사2000 데이터베이스에서 18세에서 90세 6,525명의 자료를 선별하여 지역(대도시, 중소도시, 농촌)에 따라 구분하였다. 구강검사는 세계보건기구(WHO) 기준에 따라 실시하였고, 치주 상태는 지역사회 치주지수(CPI)에 근거하여 결정하였다. 다중회귀분석을 통해 지역적 요인과 치주 상태와의 관련성을 분석하였으며, 통계 분석은 SAS 9.1.3을 이용하였다. 연구결과. 지역에 따른 치주염 유병률은 대도시, 중소도시, 농촌에서 각각 21.4%, 30.0%, 32.6%로 나타났다(p<0.01). 지역적 요인과 치주 상태와의 관련성을 살펴본 결과 다른 지역과 비교하여 농촌에서 치주염 유병률이 높은 것으로 나타났으나 (OR=1.776, 95% CI (1.562, 2.020), 사회경제적 요인, 주관적 건강 인식, 건강 관련 행동, 전신질환 유무의 혼란 변수를 보정한 결과, 중소도시에서 가장 높은 것으로 나타났다(OR=1.218, 95% CI (1.026, 1.446)). 결론. 우리나라 성인의 치주 상태는 거주 지역 요인과 관련이 있는 것으로 나타났으며, 본 연구 결과는 구강건강의 지역적 불평등 해소를 위한 대책 마련에 기여할 수 있을 것으로 기대한다.
