Comparison of nutritional status by energy level of night snack in Korean adults: using the data from 2005 Korean National Health and Nutrition Examination Survey

한국 성인의 야식의 에너지 수준에 따른 영양상태 비교: 2005년 국민건강영양조사 자료 이용

Abstract

This study aims to investigate the nutritional status of night eaters using the data from 2005 Korean National Health and Nutrition Examination Survey. A total of 3,903 subjects aged 20 and above were divided into 3 groups by using 24-hr recall data according to the night snack calorie intake: non-night snack, night snack less than 500 kcal and 500 kcal and more. Their data were analyzed to find out the difference on the socio-demographic, anthropometric, blood pressure, blood parameters and dietary characteristics. Among the subjects, non-night eaters were 66.0%, night eaters of less than 500 kcal were 28.4% and 500 kcal and above were 5.6%. Male adults, young-aged, higher educated, higher income earner, breakfast skipper and frequent dine-outer (3 times and more a day) were found more in the night eaters with 500 kcal and above. Night eaters above 500 kcal showed higher waist circumference, Glu-FBS, Glu-PP120 and also showed higher daily intake of fat and alcohol per 1,000 kcal and food groups of meat & eggs, beverages and alcohol drinks (p < 0.05), while they showed the lowest carbohydrate energy ratio of $58.3{\pm}13.7$ among the three groups. These results suggest that habitual night snack intake above 500 kcal could lead to abdominal obesity and diabetes due to higher intake of meat, fat and alcohol.

야간에 고열량 식품 섭취는 그대로 체지방으로 축적되어 비만을 유발한다고 한다. 본 연구는 2005년 국민건강영양조사 자료를 이용하여 20세 이상의 성인 3,903명 (남자 41.6%, 여자 58.4%)을 대상으로 야식의 에너지 수준을 야식 비섭취군, 500 kcal 미만 야식군, 500 kcal 이상 야식군의 3군으로 분류하여 체위, 혈압 및 혈청성분, 식습관, 식품 및 영양소의 섭취상태를 비교하였다. 1) 전체대상자 중 야식을 섭취하는 사람은 34.0%이었고, 500 kcal 미만 야식군은 28.4%로 500 kcal 이상 야식군의 5.6%에 비해 약 5배 정도 많았다. 500 kcal 미만 야식군은 여자가 남자보다 많은 반면, 500 kcal 이상 야식군은 남자가 여자 보다 약 3.9배 정도 많았고 젊은 연령대와 대졸 이상의 학력자 및 소득 수준이 높은 사람이 많았으며, 자가진단한 건강상태가 좋지 않다고 답한 사람이 적었다. 2) 평균 체질량지수는 군 간에 차이가 없었으나, 평균 허리둘레는 500 kcal 이상 야식군에서 다른 두 군보다 높게 나타났고, 성과 연령을 보정한 후에도 허리둘레와 야식의 에너지 섭취량 간에 약한 양의 상관성을 나타내었다. 3) 이완기 혈압, 공복 혈당, 혈청 중성지방, 혈청 GPT, 혈청 크레아티닌의 농도는 500 kcal 이상 야식군에서 다른 두 군보다 높게 나타났으며, 야식의 에너지 섭취량과의 상관분석 결과 공복혈당과 경구당부하 2시간 후 혈당만이 야식 에너지 섭취량과 약한 양의 상관관계를 나타내었다. 4) 500 kcal 이상 야식군에서 아침 결식자가 많았고, 외식 횟수가 하루 3회 이상인 사람이 다른 두 군에 비해 약 2배 정도 높게 나타난 반면, 야식 비섭취군에서 간식을 안하는 비율이 높았다. 5) 500 kcal 이상 야식군에서 육 난류, 음료류, 주류, 패스트푸드류의 섭취 빈도가 다른 두 군에 비해 높게 나타났고, 영양소 섭취량을 섭취 에너지 1,000 kcal당으로 환산한 결과는 지방과 알코올의 섭취량이 높게 나타난 반면 탄수화물, 조섬유, 칼슘, 인, 철, 나트륨의 섭취량이 낮았다. 식품군 섭취량과의 상관분석 결과에서 육 난류, 음료류, 주류, 과일류, 우유 및 유제품류가 야식 에너지 섭취량과 양의 상관관계를 나타내었고, 영양소 섭취량에서 칼슘, 인, 철, 칼륨, 조섬유와 약한 음의 상관관계를 나타내었다. 6) 500 kcal 이상 야식군은 다른 두 군에 비해 탄수화물 섭취 비율이 낮은 반면 지방의 섭취 비율은 높게 나타났다. 이상의 연구결과로 부터 야식의 에너지 수준이 500 kcal 이상인 경우 주로 동물성 식품, 고지방 식품, 주류 및 음료류 등 고열량 식품군의 섭취가 높고, 허리둘레, 공복 혈당, 경구 당부하 2시간 후 혈당이 높아 야식의 빈번한 섭취가 한국 성인, 특히 젊은 층의 남성에서 복부비만과 당뇨병을 야기할 가능성을 제시하였다.
