A Study on the Status and Problems of Formal Caregiving System by Hospitals in South Korea

Abstract

Purpose: This study was conducted to provide basic descriptive information on formal caregivers working in South Korea and compare their working conditions by types of medical institution. Research Method: A cross-sectional formal caregiver survey was carried out and data were obtained from hospitals enrolled in the National Health Insurance Cooperation. Among 1,584 hospitals, total 575 hospitals that use formal caregivers were analyzed. Results: About 14.7 % of inpatients in tertiary hospitals used formal caregivers during their hospitalization, while 13.2% in general hospitals and 18.0% in local hospitals. There were statistically significant differences by classifications of hospital in formal caregiver management, types of work, and working conditions (p<.05). Interestingly, the levels of continuing education or training, organizational supports, working conditions for formal caregivers in tertiary hospitals were lower than that of local hospitals. Also, patient-reported problems with formal caregivers, such as uncomfortable relationship, unfriendly manner, disclosing private matters, and lack of fundamental knowledge, were occurred more frequently in tertiary hospitals than in local hospitals. Conclusion: Current formal caregiving system contains potential risks to patients because of disconnection between basic and advanced nursing care. We recommend nursing team approach to deliver safe, holistic, and effective nursing services.

본 연구는 의료기관종별에 따른 간병인 관리현황 및 문제파악을 위해 2010년 8월 1일부터 11월 15일까지 설문조사를 통해 병원급 이상 1,584개 의료기관 전수를 대상으로 하였으며, 최종 분석은 상급종합병원의 경우 44개소 중 국민건강보험공단의 간병서비스 시범병원 2개소를 제외한 42개소 전수, 종합병원 162개소, 병원 371개소로 하였다. 연구 설계는 상급종합병원, 종합병원, 병원의 간병인 관련 부서인 원무과 및 간호부를 대상으로 병원 관련 현황 및 유료 간병인 이용률, 간병인 관리와 문제점에 대한 현황을 조사한 서술적 조사연구이다. 분석결과 입원환자의 간병인 이용률은 상급종합병원 14.7%, 종합병원 13.2%, 병원 18.0%였으며, 기관당 간병인수는 상급 종합병원 115.6명, 종합병원 34.9명, 병원 19.7명이었다. 의료기관종별에 따른 간병인 관리 현황 및 복지, 간병인 활동 관련 문제들은 유의한 차이가 있는 것으로 나타났으며, 특히 병원에 비해 상급종합병원은 간병인 전담인력 배치, 간병인 교육, 간병인 복지가 낮은 수준이었으며, 간병인 활동 관련 문제에 있어서도 환자불편호소, 불친절, 비밀누설, 비위생, 기초지식부족등의 문제가 빈번히 발생하는 것으로 나타났다. 이러한 간병인 관리 현황 및 문제점을 기반으로 의료기관의 간호인력 확보와 간호 서비스 질 향상을 위한 정책 마련이 요구된다.
