The Change of Binocular Visual Function in Mild, Moderate and Severe Glaucoma Patients

Abstract

Purpose: To assess binocular vision changes in primary open angle glaucoma (POAG) patients with severity of glaucoma. Materials and Methods: Forty five subjects with POAG were divided according to mean deviation (MD) into mild ($-1.90{\pm}1.58$ dB, $45.0{\pm}10.8$ years), moderate ($-8.30{\pm}3.07$ dB, $45.6{\pm}10.2$ years) and severe ($-14.1{\pm}1.70$ dB, $47.8{\pm}11.2$ years) group. All glaucoma subjects and age-matched normal subjects underwent binocular visual field testing; phoria angle, NPC (near point of convergence), fusion amplitude, sensory suppression and stereoacuity. All measurements among groups were statistically compared with the SPSS 21.0. Results: The patients with mild glaucoma showed significantly increased in near exophoria angle, increased in NPC, decreased in near positive relative convergence, near positive relative convergence and stereoacuity. With the progression of the glaucoma, in the patients with moderate glaucoma decreased far fusional reserve were found and in the patients with severe glaucoma increased far exophoria angle, decreased far positive total convergence, far and near sensory suppression were found, additionally. Conclusion: Visual functions related to near vision and stereoacuity were decreased in early glaucoma patients and visual functions related to far vision, sensory suppression were additionally decreased with the severity of glaucoma.

목 적: 원발개방각 녹내장 환자에서 녹내장의 진행단계에 따른 양안시기능을 평가하였다. 방 법: 안과에 내원하여 전문의에게 원발개방각 녹내장 진단을 받은 환자 45명을 대상으로 하였으며, 시야검사 결과에 따라 녹내장 초기군($-1.90{\pm}1.58$ dB, $45.0{\pm}10.8$세), 중기군($-8.30{\pm}3.07$ dB, $45.6{\pm}10.2$세), 말기군($-14.1{\pm}1.70$ dB, $47.8{\pm}11.2$세)으로 세분하였다. 녹내장군과 유사 연령대의 정상대조군을 대상으로 양안시기능검사(사시각, 폭주근점, 융합능력, 억제, 입체시 검사)를 시행하였고, 검사결과는 SPSS 21.0을 이용하여 분석하였으며 유의 수준은 p<0.05로 하였다. 결 과: 녹내장 초기에는 근거리 사시각이 외편위 방향으로 증가하였고, 폭주근점의 증가, 근거리 양성전폭주의 감소, 근거리 양성상대폭주의 감소 및 입체시 감소가 나타났으며, 중기에는 원거리 폭주여력이 감소하고 말기에서는 원거리 사시각의 외편위 방향 증가, 원거리 양성전폭주의 감소, 원거리와 근거리 억제가 나타났다. 결 론: 초기 녹내장 환자에서는 근거리 양안시기능과 입체시가 저하되고, 녹내장이 진행되면서 말기에는 억제 및 입체시가 더욱 저하되고 원거리 양안시기능이 저하되는 것을 확인하였다.
