Factors Affecting the Occurrence of Chronic Kidney Disease in Adults : Secondary Analysis Using Data from the 7th Korea National Health and Nutrition Examination Survey (KAHANES, 2016)

성인의 만성콩팥병 발생에 영향을 미치는 요인 및 위험도 : 제7기 2016년도 국민건강영양조사를 활용한 이차 자료분석

Abstract

This study is a secondary data analysis study to investigate factors and risk factors of chronic kidney disease using data of the first year (2016) of the 7th national health and nutrition survey. The extracted data were analyzed using the Rao-Scott chi-square test, the composite sample normal linear model t-test, and the composite sample logistic regression model by calculating the estimated glomerular filtration rate. The general characteristics affecting the development of chronic kidney disease were age, household monthly income, education level, occupation, personal insurance, and disease related characteristics were hypertension, diabetes and dyslipidemia. Physiological parameters were systolic blood pressure, body mass index, fasting blood sugar, glycated hemoglobin, high density lipoprotein cholesterol, hemoglobin, hematocrit and red blood cell count. Therefore, it is necessary to utilize the estimated glomerular filtration rate routinely for the screening test. A multidisciplinary self-management program is needed to improve health knowledge, dietary habits, lifestyle habits, coping abilities and self-care, taking into account general and disease-related characteristics and physiological indicators that have been identified as factors affecting chronic kidney disease. It is also necessary to establish a systematic strategy that can be applied in the public healthcare and the private healthcare for the early detection, early recognition, and early intervention of chronic kidney disease.

본 연구는 국민건강영양조사 제7기의 1차 년도(2016년) 자료를 이용하여 만성콩팥병 발생에 영향을 미치는 요인과 위험도를 파악하기 위한 2차 자료 분석연구이다. 연구 대상은 층화집락표본추출 방법을 이용하여 전국 단위로 표집되었다. 추출된 자료는 추정사구체 여과율을 산출하여 복합표본설계 Rao-Scott 카이제곱검정, 복합표본 일반선형모형 t-검정, 복합표본 로지스틱 회귀모형을 이용하여 분석하였다. 만성콩팥병 발생에 영향을 미치는 일반적 특성 요인은 연령, 가구월소득, 교육정도, 직업, 개인보험 가입여부였으며, 질병관련 특성으로는 고혈압, 당뇨병, 이상지질혈증, 생리적 지표로는 수축기 혈압, 체질량지수, 공복혈당, 당화혈색소, 고밀도지질단백질 콜레스테롤, 헤모글로빈, 헤마토크리트, 적혈구수가 영향요인으로 나타났다. 따라서 만성콩팥병의 선별검사를 위해 추정사구체여과율을 일상적으로 활용하고, 조기발견, 조기인식, 조기중재를 위하여 영향요인으로 밝혀진 일반적 질병관련 특성과 생리적 지표를 고려하여 건강지식, 식이습관, 생활습관 파악 및 대처능력과 자가간호를 증진할 수 있는 다학제간 자기관리 프로그램을 개발하여 공공의료 부문과 민간의료 부문에서 적용할 수 있는 체계적인 전략 구축이 필요하다.
