The impact of graphic health warning labels on cigarette packs on smoking attitude among Korean adolescents

Abstract

Objective: This study aimed to evaluate the impact of exposure to graphic health warning labels on cigarette packs on smoking attitude among adolescents. Methods: This study used the data of 62,276 subjects which were participated in the 13th (2017) Korea Youth Risk Behavior Web-based Survey. We conducted frequency analysis, chi-square test and multinomial logistic regression analysis using SPSS 24.0 Program. Results: After implementing the graphic health warning labels on cigarette pack on December 23 2016, about 70% of students had seen graphic health warning labels during the past one month. Among adolescents who reported experience a graphic health warning labels, recognition of negative health effects of smoking was higher in non-smokers (aOR=5.12, 95% CI=4.68-5.60) than current smokers. Also, non-smokers (aOR=10.16, 95% CI=9.28-11.11) showed a significantly higher motivation not to smoke than current smokers. Conclusion: Graphical health warning labels on cigarette packs motivate adolescents not to smoke, particularly among non-smokers. Therefore, this study suggested that development of an effective pictorial warning is needed for smoking cessation promotion and change of attitude toward smoking for youth smokers.

연구목적: 이 연구의 목적은 2016년 12월 23일부터 도입된 담뱃갑 경고그림 정책 효과를 살펴보기 위한 것으로 청소년의 경고그림 노출 여부 및 이로 인한 흡연에 관한 태도 및 인식에 미친 영향을 흡연 상태에 따라 탐색해 보는 것이다. 연구방법: 본 조사는 2017 청소년 건강행태 온라인 조사를 활용하여 전국 62,276명 청소년 자료를 기반으로 현재 흡연여부 및 담뱃갑 경고그림 노출 여부를 파악하고 경고그림으로 인한 흡연 폐해 인식 및 비흡연 의지가 흡연 상태에 따라 달라지는지를 다항 로지스틱 회귀분석을 실시하여 보정 교차비(aOR: Adjusted Odds Ratio)와 95% 신뢰구간(CI: Confidence Interval)을 산출하였다. 연구결과: 경고그림 도입 후 약 6개월 경과 후 청소년의 약 70%는 경고그림을 본 적이 있는 것으로 나타났다. 경고그림을 본 후 흡연이 건강에 해롭다고 생각할 가능성은 현재 흡연자에 비해 비흡연자일 경우 5.12배 (aOR=5.12, 95% CI=4.68-5.60) 높았고, 담배를 피워서는 안 되겠다고 생각할 가능성 또한 흡연자에 비해 비흡연자일 경우 10.16배 (aOR=10.16, 95% CI=9.28-11.11) 높았다 결론: 현재 시행되고 있는 담뱃갑 경고그림의 노출률은 높고, 경고그림이 청소년의 흡연 인식 및 금연 의지에 긍정적인 영향을 미친 것으로 나타났으며, 이는 현재 흡연 청소년에 비해 현재 비흡연 청소년의 인식 및 의지 강화에 효과적이었다. 향후 흡연 청소년들의 태도 및 행동 변화를 유발할 수 있도록 계속해서 효과적인 경고그림이 개발하여 청소년들의 흡연을 사전에 예방하고 청소년의 흡연율 감소를 도모해야 할 것이다.
