난소의 성숙 낭성 기형종에서 발생한 육종 1예

Abstract

=성숙낭성기형종은 전체 난소종양의 약 20%를 차지하는 흔한 종양으로 성숙낭성기형종의 약 10%는 폐경 이후 발생한다. 약 1-2%에서 악성 변형을 동반하며 이러한 악성 변형의 대부분이 폐경 여성에서 나타난다. 동반되는 악성 변형으로는 편평상피세포암이 가장 흔하며 (75% 이상), 그 외 선암종, 육종, 흑색종, 역형성암종 등이 드물게 보고되고 있다. 성숙낭성기형종에서 악성 변형된 육종은 매우 드물기 때문에 임상병리학적 특성이 아직 확립 되어있지 않으며, 적절한 치료법이나 예후 또한 모호한 상황이다. 따라서 아직까지 종양표지물질이 밝혀지지 않았으며 수술 후 보조적 치료에 대한 효과 및 방법 또한 증명된 바가 없다. 본 저자들은 성숙낭성기형종에서 기인한 육종 1예를 경험하였기에 간단한 문헌 고찰과 함께 보고하는 바이다.
