Factors Influencing Anxiety and Depression in Breast Cancer Patients Treated with Surgery

Abstract

Objectives : The purpose of this study was to investigate the variables influencing the anxiety and depression in the patients with breast cancer with surgery. Methods : Forty nine outpatients with breast cancer were participated from the breast center of a Dong-A university hospital. Socio-demographic characteristics, clinical variables, anxiety, depression, self-esteem, body image, quality of life were evaluated. Results : Correlation analysis showed several variables were associated with the anxiety and depression of the breast cancer patient. Body image(r=0.57, p<0.01), side effect of systemic therapy(r=0.72, r<0.01), breast symp-tom(r=0.56, p<0.01), arm symptom(r=0.58, p<0.01), upset by hair loss(r=0.37, p<0.01), self esteem(r=-0.36, p< 0.05) and future perspective(r=-0.52, p<0.01) were associated with score of Hamilton Anxiety Scale(HAS). Body image(r=0.62, p<0.01), side effect of systemic therapy(r=0.70, r<0.01), breast symptom(r=0.61, p<0.01), arm symp-tom(r=0.67, p<0.01), upset by hair loss(r=0.32, p<0.05), future perspective(r=-0.52, p<0.01) were associated with score of Hamilton Rating Scale for Depression(HRSD). According to stepwise linear regression analysis on the breast cancer patients of HAS total score, the following two variables were founded : side effect of systemic therapy(p<0.01) and body image(p<0.05). With HRSD total score, the following three variables were founded: side effect of systemic therapy(p<0.05), body image(p<0.05), and arm symptom(p<0.05). Conclusion : Psychological factors such as body image and self-esteem were also important as socio-demographic and clinical characteristics in depression and anxiety of patients with breast cancer. For depression and anxiety in patients with breast cancer, development of systematized program and appropriate psychological intervention are needed.
