Development of Disease Knowledge Instrument for Patients with Chronic Hepatitis B

만성 B형간염 환자의 질병관련 지식 측정도구 개발

Abstract

The purpose of this study was to develop a valid and reliable instrument for measuring knowledge of disease in patients with chronic hepatitis. Sixty-three preliminary items in 10 domains were identified through literature review with 55 items having over 0.80 of content validity index in modified 8 domains. After a pilot test, a survey was conducted with 280 patients to confirm both the reliability and validity of the instrument. For corrected item-total correlation coefficient that has been developed from the 49-question preliminary instrument, excluding 6 questions with low item-total correlation, Cronbach alpha was found 0.877. The results of measuring the knowledge score at a 2-week interval in patients with chronic hepatitis B showed the confirmation of a strong positive correlation, supporting safety-reliability. Construct validity was supported through confirmatory factor analysis and a comparison of knowledge scores. Patients who had received hepatitis B education previously scored significantly higher on disease knowledge than their counterparts (p<.001). Knowledge scores between nurses and patients, which showed significant difference between the two groups (t=-5.524, p<.001). In conclusion, knowledge of disease for patients with hepatitis B can be measured with the instrument.

본 연구는 만성 B형간염 환자에게 적용 가능한 질병관련 지식 정도를 확인할 수 있는 신뢰성있고 타당성 있는 도구를 개발하기 위한 방법론적 연구이다. 문헌고찰 등에 따라 10개 영역의 63개의 예비문항을 작성하였고, 전문가 집단의 내용타당도 검증과 예비조사를 통하여 CVI 0.80 이상인 8개 영역의 55개 문항의 예비도구를 개발하였다. 예비도구는 총 280명의 만성 B형간염 환자에게 설문조사를 하여 타당도 및 신뢰도를 검증하였다. 수정된 문항-합계와의 상관계수가 낮은 문항 6개를 제외하여 최종적으로 49문항으로 결정하였다. 내적 신뢰도 Cronbach alpha=.877이었고, 만성 B형간염 환자를 대상으로 2주 간격으로 지식점수를 측정한 결과 강한 정(+)의 상관 관계가 확인되어 안전성 신뢰도가 지지되었다. 구성타당도는 요인분석과 집단비교를 통해 확인하였다. 확인적 요인분석 결과 문항별 적재값이 .58이상으로 나타났다. 또한, 만성 B형간염 환자의 교육 유무에 따른 질병관련 지식점수를 평가한 결과 유의한 차이가 있었으며(p=.033) 간호사집단과 환자집단의 지식점수도 유의한 차이가 확인되어(t=-5.524, p<.001) 구성타당도가 지지되었다. 최종 도구로 조사된 정답률은 문항에 따라 14.9-93.2% 범위이고 평균정답률은 58.5%이었다. 개발된 도구는 만성 B형간염 환자의 질병관련 지식 정도를 측정할 수 있으며, 건강관리자들은 이를 바탕으로 자가 간호 수행을 증진할 수 있는 프로그램을 구성하고 효과를 확인하는데 유용하게 사용할 수 있다.
